check the usb settings in the bios. That happened to me because I didn't have the EHCI HAND-off. besides, i also had to disable usb 3.0 ports and enable usb in legacy. It seems to me that all your problems are related to the configuration of the bios.

my bios havent ehci, ill try to off usb 3.0 ports
